Job Description

We are seeking an experienced

Behavioral Health Consultant

to join their integrated primary care team. This role focuses on supporting patients with both medical and behavioral health needs, while collaborating closely with primary care providers and other clinical staff to ensure a holistic approach to care.

Key Responsibilities

Partner with primary care and behavioral health teams to address mental health and behavioral concerns across all age groups -- children, adolescents, adults, and families. Deliver short-term, solution-focused therapy and help patients develop personalized behavior change plans. Educate patients on self-management strategies to promote emotional and behavioral wellness. Support and train primary care providers in the use of behavioral health techniques and interventions. Promote the organization's integrated care model to strengthen collaboration between behavioral health and medical services. Participate as an active member of a multidisciplinary team, contributing to improved patient outcomes and care coordination. Maintain documentation and provide services that align with organizational standards, policies, and best practices. Communicate effectively and respectfully with patients, families, and team members to support high-quality, patient-centered care.

Qualifications

Master's degree in Psychology, Counseling, Social Work, or a related behavioral health field. Must hold an active

Ohio license

as an

LISW, LPCC, or Psychologist

. Minimum of two years of independent practice experience. Strong interpersonal and communication skills with the ability to work both autonomously and as part of a collaborative care team. Experience with electronic medical records (EMR) systems preferred. Comfortable working with diverse populations and flexible with scheduling needs.
Job Type: Full-time

Pay: $79,656.43 - $95,930.32 per year
