Job Description

Under the supervision of the Medical Director, the Behavioral Health Clinician provides outpatient counseling and therapeutic services as part of our whole-person healthcare model. The Clinician is responsible for intake assessments, treatment planning, individual and group counseling, and care coordination for assigned clients. This role is an integral part of our expanding behavioral health services and works closely with urgent care, primary care, and community partners to ensure clients receive comprehensive and coordinated support.

Description




ESSENTIAL DUTIES

Manage a client caseload within the outpatient behavioral health clinic. Provide supervision for unlicensed interns as needed. Conduct intake assessments, including biopsychosocial evaluations. Collaboratively develop, implement, and update individualized treatment plans and goals. Provide individual, family, and group counseling using evidence-based and culturally responsive practices. Facilitate therapeutic groups and psychoeducation sessions. Document all clinical encounters, treatment progress, and case management notes in the electronic health record in a timely and compliant manner. Provide crisis intervention and stabilization support appropriate to an outpatient setting. Coordinate care and make referrals to psychiatry, primary care, urgent care, social services, and other community resources as needed. Provide and receive warm handoffs between urgent care, primary care, and behavioral health services within Transformation Health Network to support continuity of care and whole-person treatment. Collaborate with schools, courts, probation/parole, and other external partners when client consent or court order applies. Attending team meetings, interdisciplinary case conferences, and clinical supervision. Engage in regular training and professional development to support high-quality, whole-person care. Perform other related duties as assigned or needed to support clinic operations.

EDUCATION REQUIRED

Master's degree in psychology, social work, marriage and family therapy, counseling, or related behavioral health field.

LICENSES/CERTIFICATIONS REQUIRED

Current licensure with the State of Hawaii DCCA PVL (Department of Commerce and Consumer Affairs Professional Vocational Licensing) in good standing. Accepted licenses include: LMFT, LCSW, LMHC, or equivalent. The following are required for clearance and may be obtained after hire: Background check Current CPR/First Aid certification TB clearance

EXPERIENCE REQUIRED

Minimum three (3) years of experience providing counseling, therapy, or related behavioral health services. Outpatient behavioral health or integrated care experience preferred.

KNOWLEDGE REQUIRED

Strong understanding of mental health conditions, co-occurring substance use, trauma, abuse, and special populations. Familiarity with addiction screening, treatment, and referral practices. Knowledge of integrated care models and interdisciplinary collaboration in healthcare. Understanding of HIPAA, state and federal laws, ethical standards, and best practices for outpatient behavioral health. Proficiency with Microsoft Office and electronic health record systems.

Ability to communicate clearly and effectively in English, both verbally and in writing. Strong assessment, treatment planning, and clinical documentation skills. Crisis management skills with a focus on de-escalation and outpatient stabilization. Compassion, empathy, adaptability, and cultural sensitivity. Ability to collaborate effectively across disciplines (primary care, urgent care, behavioral health). Commitment to whole-person, client-centered care. Bilingual (Spanish/English or other language) a bonus.

Clinical office, well lit, air-conditioning. All staff are expected to keep their work areas clean and free of clutter, as well as maintaining the break room by cleaning up after themselves, and occasionally serving one another by cleaning surfaces, putting dishes away, etc.

PHYSICAL DEMANDS

Work is primarily sedentary with periods of walking and standing required. Ability to lift up to 15 lbs. occasionally, and up to 25 lbs. with assistance. Adequate hearing, vision, and speech (with or without correction) to carry out job duties. POSITION WAGE AND BENEFITS


Hourly position starting at $65/hr with opportunity for growth based on program development, merit and benchmarks with the company.
