About the Role

The

Nurse Specialist - Triage Lead

is a key member of our care team, providing high-quality telephone triage, patient education, and care coordination. In addition to clinical responsibilities, you'll lead triage operations, train staff, audit workflows, and ensure safe, efficient, patient-centered care. This role is perfect for a nurse who thrives in leadership and collaboration.

What You'll Do

Clinical & Triage:

+ Serve as a trusted resource for patients and families with clinical questions
+ Triage acute medical issues using established protocols
+ Communicate test results and care instructions per provider direction
+ Provide patient education and self-management support
+ Document all interactions accurately in the EMR

Leadership & Team Support:

+ Oversee triage workflows and daily huddles
+ Train and mentor triage nurses
+ Conduct chart audits and ensure compliance with protocols
+ Assist with scheduling, evaluations, and team development

Care Coordination:

+ Track patients after hospital or ER visits
+ Educate patients on care plans and assist with goal setting
+ Connect patients to internal and external resources for social or financial needs

What We're Looking For

Current

RN license in Kentucky

CPR certification Education & Experience: + Nursing Diploma + 3 years experience OR
+ ADN + 5 years experience OR
+ BSN + 1 year experience OR
+ MSN + 1 year experience
Strong communication, leadership, and organizational skills Passion for patient-centered care and team collaboration
