Job Description


The Lead Direct Care Professional is a key associate at Hope Network whose primary responsibilities include:

  • Provide oversight, support and care for consumer\xe2\x80\x99s of Hope Network\xe2\x80\x99s service in a variety of settings;
  • Role model appropriate behavior and skills to consumers to help them achieve their highest level of independence;
  • Use a consumer\xe2\x80\x99s prescribed plan(s) in support of their medical, clinical, therapeutic and behavioral needs.
  • Ability to mentor, train and orient new staff
  • Provide leadership, direction and support to staff on shift. Go to person when supervisor is not available or on-site.
Rover Pool (Float) - Providing direct care coverage in varying facilities on a variable schedule to ensure the highest quality of service is provided to persons served. Travel to various sites on an ongoing basis.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES
  • Regular and predictable attendance is an essential requirement of this position. Successfully complete all required and applicable trainings and in-services including annual refresher and appropriate intervention techniques.
  • Provide leadership, direction and support to staff on shift in coordination with the supervisor. This may include, but not be limited to scheduling, training, compliance, on-call pager, and administrative supports.
  • Elevate employee relations concerns to supervisor. May assist supervisor on special projects.
  • Provide input as requested in the development of the consumer\xe2\x80\x99s treatment plans and goals. Confirm treatment plans are implemented properly.
  • Implement and participate in the clinical and behavioral goals for each consumer using approved methods of instruction and intervention.
  • Provide through, accurate and on-time daily observational data and document using electronic record according to Agency standards.
  • Safely transport consumers, report any vehicle maintenance needs, and have an accident (preventable) free driving record according to Agency standards.
  • Communicate and record health concerns, administer and document prescribed medications (including subcutaneous injections as trained) and treatments as prescribed/directed by nurses and physicians in accordance with Agency standards.
  • Intervene in any crisis situation that arises according to established procedures. Physical intervention should be the last-resort method. Promote safety of all individuals involved.
  • Provide direct care for individuals who are unable to complete self-care tasks independently, including but not limited to: bathing, hygiene, skin care, toileting, bowel and bladder programs, feeding (including G tubes), dressing and transfers.
  • Maintain a safe working environment by: using proper lifting techniques, positioning people safely, safely securing people in their wheelchairs and addressing safety issues before an accident occurs. Also responsible for reporting any accidents, filling out incident and basic injury reports as needed in a timely manner. Ensure safety measures are implemented appropriately.
  • Has thorough knowledge of fire and smoke alarm systems and all emergency evacuation procedures according to agency standards.
  • Promote a positive morale among staff by modeling and encouraging effective teamwork and communications. Continually work to promote successful working relationships among all co-workers; alert supervisory staff of situations that may require further intervention.
  • Develop and maintain healthy, professional relationships with boundaries with consumers and co-workers while providing consistent follow through and enforcing applicable rules and routines of the home and/or program. Develop and maintain appropriate professional relationships with family/friends/guardians and outside agencies.
  • Monitor, supervise, accompany and interact with consumers in their home or program that is consistent with their person-centered plan.
For Rover Pool positions:
  • Must be able to travel / float to various sites on an ongoing basis.
  • Must be able to work a flexible schedule to accommodate the needs of the program in which you may be scheduled.
  • Valid driver\xe2\x80\x99s license, required for the Rover Pool positions.
Educational / Talent Requirements
  • Minimum Education required:
  • Bachelors degree with 2 years experience in applicable setting
  • Associates degree with 3 years experience in applicable setting
  • HS Diploma/GED with 4 years experience in applicable setting.
  • Demonstrated critical thinking/problem solving skills to respond appropriately to medical and behavioral issues.
  • Demonstrated leadership skills and ability to problem solve independently.
  • Demonstrated ability to train, orientate and mentor staff
  • Ability to work with and respect individuals from various religious, ethnic, and social-economic backgrounds.
  • Demonstrated ability to take initiative and action to respond, resolve and follow up regarding service issues with consumers/family members/guardians in a timely and appropriate manner.
  • Ability to lift up to 50lbs non-stationary weight. Ability to bend, stoop, twist, pivot and kneel while bearing weight of a consumer. Demonstrated ability to transfer individuals using proper lifting techniques. Ability to stand a majority of the eight-hour shift
  • Current, valid state driver\xe2\x80\x99s license and acceptable driving record according to Hope Network policies, preferred (Driver\xe2\x80\x99s License may be required by some programs).
  • Ability to complete all required training for the position, including annual trainings and professional development. Advanced medical trainings viewed favorably.
  • Demonstrated ability to communicate in written, verbal and electronic formats to meet position requirements. Demonstrated basic computer skills.
