:

Provides clinical leadership within the community, specialty, and ambulatory clinical pharmacy services for program development and evaluation. Assists the Manager, Leadership, and caregivers in the development, assessment and provision of clinical pharmacy services to ensure alignment with departmental and organizational objectives. Area of focus for this position includes: immunization clinical programs, travel health services, MTM, pharmacist prescribing, medication administration, and PIC regulatory compliance.

Required Qualifications:

• Education to meet certification, license, or registration requirement.
• Oregon Pharmacist License upon hire. Or
• Oregon Pharmacist Preceptor License upon hire.
• A post-graduate year residency (PGY1) or a minimum 2 years\' experience in specialty pharmacy in lieu of residency
• A post-graduate year 2 residency (PGY2) or a minimum 3 years\' experience in specialty pharmacy in lieu of residency.

The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.

About the Team: Providence Home and Community Care (HCC) is a service line within Providence with over 8,000 caregivers and 2,000 volunteers serving nearly 200,000 people a year, in their homes, in clinics and in a variety of congregate living ministries across our seven-state footprint.

HCC provides a full range of services and support for people of all ages, especially those facing chronic or life-limiting illnesses.
