Qualified candidates must possess a Bachelor's Degree or equivalent experience at the time of hire plus three years of case management experience. Two years of experience may be substituted with a Bachelor's Degree. Education can be waived with four years of experience. The candidate must have experience working with a high-risk population and considerable oral and written communications experience. Candidate must be able to engage and develop collaboration with community agencies, offer concrete problem solving, and engage in client skill-building. This position is a Lead Case Manager position. The facility houses up to 67 male Clients.

JOB RESPONSIBILITIES:
Must have the desire to work with offenders.
Complete client intake within seven days of program entry.
Assess and evaluate client needs.
Will provide case management services to clients.
Schedule case management sessions and complete required case notes.
Develop and maintain community resources.
Conduct PREA Risk Assessment.
Develop Individualized/Treatment Plans for all clients.
Ensure security and confidentiality and uniformity of content and management of all client records. Provides timely and accurate file and database documentation. Keep records up to date at ALL times.* Keep client records up to date and accurate at all times.
Maintain ongoing communication with referral agents. Prepares court reports, monthly reports, and accurate and timely discharge. Prepare all required reports - Incident reports, monthly reports, etc.
Work with clients to secure basic needs (i.e., medical insurance, housing, education, state assistance) and other community support systems through "counseling by walking." Advocate on behalf of clients to ensure quality services, substance abuse treatment, and stable housing if needed.
Will conduct a pat-down search on male clients upon entering the facility.
Will monitor urinalysis on male clients.
Monitor client behavior while holding the client accountable for their actions.
Model pro-social behaviors and serves as a role model to clients and staff. Maintains professional relationships with clients
Remain current on research and literature on "best practices" for working with criminal justice populations. Attend all staff meetings and required training.
Transport clients in program vehicles, as required.
Be responsible for a caseload of up to 20 plus clients.
Will perform other duties as required.
