Kingsbridge Heights Community Center (KHCC) is a 50-year-old, multi-service, not-for-profit organization based on the settlement house model, located in the Bronx. We provide resources for Bronx residents from cradle to career to advance education and well-being for a vibrant community. KHCC serves over 5,000 families annually with an inclusive environment for children with special needs, low-income families, recent immigrants, survivors of sexual and domestic abuse, and at-risk teens. KHCC helps participants overcome adversity and develop the skills they need to build bright futures.
The KHCConnect Case Manager will be responsible for both supporting the KHCConnect program and adult participants with case management and direct service, as well as completing administrative tasks related to contract compliance and documenting service history.
Specific Duties and Responsibilities:
? Conduct initial assessments with new and returning adult participants using the Universal
Screening Form (USF)
? Follow-up all in-house screenings with a Medicaid eligibility check, and (as possible)
screening/navigation through the 'Whole You' Social Care Network (SCN) behavioral
health care initiative.
? Provide consistent case management, advocacy, and internal referrals to other KHCC
programs as well as outside referrals to students who present with needs beyond the
scope of KHCC's service continuum.
? Support participants via direct services, such as SNAP and Housing Connect
applications, crisis intervention, distribution of basic need items, gift cards and micro
grants etc.
? Support the planning and implementation of community-centered and civic engagement,
advocacy or agency events and workshops, etc.
? Participate in weekly team meetings, supervision, and ongoing training opportunities.
? Engage and retain participants in KHCC programming by developing supportive
relationships
? Document each interaction with students in Salesforce in a timely manner; support
program data entry
? Other duties as assigned by Adult Services leadership team
Qualifications:
? Must be authorized to work for any employer in the US
? Willing to undergo background checks in accordance with federal, state and local laws
? Bachelor's degree and/or some experience in human services-related fields expected.
BSW preferred
? Bilingual Spanish required
? Flexibility with schedule to accommodate collaboration across the Adult Services
Department
? Exceptional organizational, follow-through, and time management skills
? Comfort with multiple computer platforms and database systems is required.
? Strong oral and written communication skills
Schedule:
? 35 hours /week, Mo/Tue/Fri: 10:00 am - 6:00 pm, Wed/Thu: 11:00 am - 7:00 pm OR
Tue/Fri: 10:00am - 6:00pm, Wed/Thu: 11:00 am - 7:00 pm, Sat: 9:00 am - 5:00 pm
Physical Demands:
? While performing the duties of this job, the employee is regularly required to stand; walk; sit; handle, or feel; talk, hear, smell or taste. On rare occasions it may be necessary to move quickly, run, etc. The employee frequently is required to use hands to finger, handle, or feel; reach with hands and arms. The employee is occasionally required to sit; stoop, kneel, crouch, or crawl. The employee must regularly lift and/or move up to 10 pounds, frequently lift and/or move up to 25 pounds and occasionally lift and/or move up to 50 pounds. Specific vision abilities required by this job include close vision, distance vision, and ability to adjust focus.
