Creative Treatment Options is an outpatient substance abuse counseling firm that has been in operation in the Denver area for over 25 years, working with mainly court-involved individuals.
We are seeking 1 full-time experienced case manager to work with us on our Jail Based Behavioral Health Services (JBBS) contract with Jefferson County to help us deliver first-class case management to incarcerated individuals taking part in our treatment program. The job will take place within the Jefferson County Jail and that is where you will provide most of your case management services. The other portion of time you will be responsible for shopping for the supplies we give to the clients when they are released, which can include a lot of different necessary items from backpacks to cell phones.
Duties include:
Conduct comprehensive assessments of clients' needs and develop individualized service plans.
Provide case management services to clients, including coordinating and monitoring their access to necessary resources and services, such as housing, health care, transportation, financial services, etc.
Maintain accurate, detailed, and up-to-date documentation of client interactions and service provisions
Collaborate with other professionals and referral agencies to ensure the delivery of coordinated and effective services. Maintaining strong relationships with referral agencies around our 3 area offices.
Monitor clients' progress towards their goals and make appropriate adjustments to service plans as needed
Experience:
Bachelor's degree in social work, human services, criminal justice or a related field
At Least 2 years of Case Management experience
Experience working in crisis management, behavioral health, or social work
Familiarity with Medicaid note writing and billing and addiction counseling is a plus
Strong communication skills, both written and verbal
Strong ability to work independently
Excellent organizational skills and attention to detail
The role of a Case Manager can vary widely from week to week, by workload and duties. We have around 60 clients that we work with in the jail program and you will be expected to meet with each of them for 10-15 minutes weekly to assess their needs and their progress towards their treatment goals.
