Wellbound for Special Needs is a Special Needs-focused Certified Home Health Agency (CHHA) dedicated to supporting children and adults with special healthcare needs and their families. We provide compassionate, coordinated, and high-quality home health services throughout New York City and Long Island.
Position Overview
We are seeking a Special Needs Intake Manager to oversee and manage the intake and admissions process for our special needs population (code 95/OPWDD). This is a full-time, in-office role based in Brooklyn. The Intake Manager plays a critical role in ensuring timely, compliant, and family-centered onboarding of clients while working closely with clinical, administrative, and referral partners.
Key Responsibilities:
Manage all incoming pediatric/adult special needs referrals from hospitals, physicians, schools, care managers, and families
Review intake documentation for accuracy, completeness, and eligibility
Coordinate physician orders, insurance verification, and authorizations to ensure timely admissions
Manage the intake team of 3-4 Intake Specialists who are the primary point of contact for Special Needs families during the intake process
Manage a team that clearly and compassionately with families regarding services, timelines, and expectations
Collaborate with nursing, scheduling, rehab, and administrative teams to ensure smooth start-of-care
Ensure compliance with CHHA, NYS Department of Health, Medicaid, Medicare, and payer requirements
Maintain accurate documentation in electronic health record (EHR) systems
Track intake metrics, referral sources, and admissions data
Participate in quality improvement initiatives related to intake and admissions
Qualifications
Required:
Minimum of 3 years' experience in healthcare intake or admissions
Experience with pediatric special needs populations strongly preferred
Knowledge of CHHA operations and insurance authorization processes
Strong organizational and communication skills
Ability to work in a fast-paced, office-based healthcare environment
Preferred.
Experience with NYS CHHA regulations and Medicaid
Bilingual (English/Spanish or other languages common in NYC)
