Job Summary

The Intake Coordinator serves as the point of contact for pre-clinical, clinical, and termination processes at the center. In conjunction with the clinical staff and center director the position provides assessments, assists in service plan developments, and other clinical processes. Assesses by phone and face-to-face contact the needs of patients and their families, collects data and assigns clinical liaison for further contact. Coordinates all aspects of referral/admission processes and compiles referral/admission statistical data. Actively involved in professional activities within the health system; participates on multidisciplinary teams and other clinical interventions to ensure individualized, family-centered health care for all patient populations seen at the family center. Communicate effectively with medical team members.

Job Responsibilities

  • To successfully perform the job of Intake Coordinator, the employee must be able to satisfactorily perform the core competencies stated above and the essential job functions listed below. The Intake Coordinator acts as the gatekeeper for patients and families who request services.
  • Coordinates, processes, responds to, and completes inquiries, referrals, pre-intake/admission, and intake/admission of potential patients and their families who request service and transfers paper work to appropriate staff to ensure proper service delivery to clients in a professional and timely manner.
  • Provides triage and assessments; performs initial telephone screening to determine patients\' appropriateness for services. Advises director and clinical staff of patient status, appropriateness of, and acceptance of service and provide all relevant information to ensure patients\' smooth transition to all clinical services.
  • Contacts referral sources to advise of referral status and updates the clinical team who will deliver the services requested.
  • Provides internal and external patient intakes and responds to consumers and stakeholders\' inquiries.
  • Keeps electronic admission and service log, protects and maintains patient confidentiality.
  • Demonstrates good problems solving and clinical skills in assessing for clinical criteria in the admission process and projecting anticipated services.
  • Provides follow up on all referrals and cases managed and documents the outcome.
  • Exchanges information with the appropriate staff and teams to identify needs and determine the Center\'s ability to meet service needs.
  • Works with staff and patients to identify potential solutions as psychosocial problems are identified for patients/families.
  • Identifies alternative community service resources when Children\'s of Alabama/CH Service\'s solutions are not available or appropriate.
  • Assists with walk-ins, unscheduled appointments, emergency coverage and crisis intervention for patients and their families as needed.
  • Assists in helping families get assigned to clinical staff based on assessments, needs, and clinical requirements.
  • Participates in client staff conferences.
  • Assures appointments and home visits are available to families and assists when appropriate.
  • Provides the work of a clinical liaison as needed.
  • Maintains good working relationships and positive public relations with all referral sources and stakeholders.
  • Participates in on-call system if required.
  • Participates in staff consultations and meetings as required or needed.
  • Maintains compliance with professional social work code of ethics.
  • Participate in In-service training, continuing education, and staff development activities.
  • Assist with the planning and participate in special events designed to promote the Center.
Education

Graduation at the Master level from an accredited School of Social Work or other Human Service related field.

Experience

Three years post graduate experience preferably in a medical setting with pediatric chronic populations. Experience in conducting psychosocial assessments and proven successful rate of working on multidisciplinary teams, customer satisfaction, and case management. Pediatric experience a plus.

Licensures, Certifications, and/or Registries

Professional license or license eligible required

Valid Alabama Driver\'s License required
