Position Summary:




We are seeking a compassionate and dedicated

Intake Coordinator

to function as a liaison to the company's centralized referral system and facilitate the provision of home care services across our coverage area. This position also assists in arranging for uninterrupted continuation of medical care of the client at home and additional services as needed for the effective care of the client.





Key Responsibilities:



Job Knowledge

: Develop and maintain a working knowledge of all services and resources provided by the Agency and those available within the community when the Agency's service lines are unable to meet the needs of the patient. Identify and verify benefits with payer sources and obtain initial authorizations and frequency of visits. Determine coverage eligibility for third-party non-governmental payers.

Communication

: Communicate with referral sources, patients, payer sources and coworkers in a positive and professional manner at all times.

Problem Solving

: Work closely with staff and patients to identify potential solutions as problems are identified with payer sources. Interact with referral sources to facilitate communications to answer questions and resolve problems.

Customer Service

: Provide excellent customer service while obtaining information from referral sources or potential patients.

Teamwork:

Work closely with supervisor, nursing team, and fiscal analysts when requests for care are inappropriate, incomplete or not a covered benefit.

Organization

: Ensure documentation and reports are complete and turned in timely. Be able to track multiple referral sources at one time.

Professional Conduct

: Maintain a positive, professional attitude and project a caring image. Establish and maintain professional relationships with hospital staff, physicians, all referral sources and Agency staff.

Professional Development

: Attend required training sessions to maintain up-to-date knowledge and skills.

Technology Use

: Utilize applications as required for recordkeeping and communication.

Other Duties

: Perform additional responsibilities as assigned by supervisors or management to meet the needs of the patient and team

Requirements:
Minimum of High School Diploma or equivalent (required). Two years of health experience preferred. Knowledge of Medicare, private insurance and Arkansas Medicaid regulations. Excellent written and verbal communication skills. Ability to make appropriate clinical judgments as it relates to the referral process. Proficient computer skills and ability to use and perform key functions of MS Office programs. * Valid driver's license.
