: The Certified Mental Health Professional utilizes clinical expertise to perform psychosocial assessment and provides quality behavioral health services including being responsible for treatment plan development, coordination, and the delivery of psychotherapy to patients and families. This position works collaboratively with patients, patients\' family/significant other, healthcare providers, insurers, community resources, and other involved parties to identify patients\' needs, broker services, coordinate care and transition plans, and advocate for patients.

This care is provided under the supervision of a licensed Clinical Mental Health Counselor (CMHC), Licensed Clinical Social Worker (LCSW), or Licensed Marriage and Family Therapist (LMFT).

  • Shift: 40 hours, 4/10\xe2\x80\x99s, evenings, rotating weekends/holidays
The Clinical Mental Health Professional will provide care appropriate to assigned patient population as established with the plan of care/treatment plan and in accordance with professional licensure. This care may take place at a hospital, facility, clinic, inpatient, outpatient or home care setting.

Related responsibilities of this role include, but are not limited to:
  • Performs social work consultation for patients and caregivers. Conducts psychosocial assessments. Under supervision of a licensed Clinical Mental Health Counselor (CMHC), Licensed Clinical Social Worker (LCSW), or Licensed Marriage and Family Therapist (LMFT), provides supportive, crisis, bereavement and other social work interventions as necessary.
  • Assess and gather sufficient information about patient\'s history and relevant biological and cultural dimensions that influence the patient, e.g. social, psychological, intellectual, spiritual, emotional and financial needs.
  • Caregiver actively works with supervising therapist to ensure that they meet the DOPL criteria for supervised hours.
  • Acts within the Scope of Practice under the Mental Health Practice Act and adheres to applicable National and State ethical standards of professional conduct for clinical profession.
  • Engages in effective and direct clinical interventions that address patient dynamics within the parameters of brief psychotherapy.
  • Able to treat patients in individual, family, and group settings.
  • Participates in the effective integration of outcome studies/measures into routine patient care.
  • Develops an active treatment plan for patient\'s current episode of care, in coordination with patient, family and other care providers in order to improve patient/family functioning.
  • Assists patients and family members in gaining insight, communication and coping skills to enable them to deal with and to understand the natural history and current episode of care and related social, psychological, financial, spiritual and emotional factors that interfere, delay or prevent the healing process.
  • Maintains on-going communication with caseworkers, attorneys, guardian and other involved parties as appropriate, in order to coordinate treatment.
  • Appropriately refers to another clinician for evaluation/consultation when necessary, or uses educational resources, various assessment tools/measures, and results of psychological testing evaluations to enhance diagnostic consultation or brief intervention focus with patients and family members.
  • Collaborates with members of the patient\'s healthcare treatment team as appropriate.
  • May coordinate the discharge planning process from patient admission to departure. Screens for and assess patient needs, develop a discharge plan, and coordinate with internal and external services for care during and after the hospital stay.
  • Provides clear and timely documentation in patient\'s chart of pertinent information related to present treatment, e.g.: assessments, interventions, phone consults, case review notes. A closing summary or transfer of care documentation is entered into the medical record when appropriate.
  • Provides appropriate supervision of residents and practicum students as assigned. If assigned to work with special populations such as eating disorder, or substance abuse and addictions, must be able to assess the patient\xe2\x80\x99s addictive or dysfunctional behaviors and related socio-cultural and biological dimensions, which influence the patient. Follows established protocol regarding drug screens.
Note: The Certified Social Worker Intern License can be utilized for up to 24 weeks (6 months) while employees are working toward preparing, testing, and passing the State Boards examination (obtaining an CSW license). After that 24 week period, or if the CSW Graduate fails the State Board exam a second time, the employee can no longer remain in the CSW-I License. These employees can apply for any open jobs they qualify for, including jobs requiring a certification the employee currently holds (i.e. SSW, psych tec, etc.). Failure to either pass boards or to be hired into another job will result in termination.

Minimum Qualifications
  • Master\'s degree in Social Work or a related field from an accredited program approved by the Utah State Board in a field relevant to license (degree will be verified).
  • Current State Licensure as a Certified Social Worker, Certified Social Worker Intern, Associate Clinical Mental Health Counselor, Associate Clinical Mental Health Counselor, LCSW, LMFT or CMHC
  • Excellent written and verbal communication skills.
  • Effective computer skills (email, word processing, and spreadsheets).

  • Experience working in a healthcare setting
  • Bi-lingual in Spanish

  • Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies.
  • Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.
  • Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, etc.
  • Expected to lift and utilize full range of movement to transfer patients. Will also bend to retrieve, lift, and carry supplies and equipment. Typically includes items of varying weights, up to and including heavy items.
  • Need to walk and assist with transporting/ambulating patients and obtaining and distributing supplies and equipment. This includes pushing/pulling gurneys and portable equipment, including heavy items. Often required to navigate crowded and busy rooms (full of equipment, power cords on the floor, etc.)
  • May be expected to stand in a stationary position for an extended period of time.
