JOB OVERVIEW:

The Inpatient Pharmacy Clinical Practice Specialist (PCPS) will work with providers, nurses, and pharmacy staff to provide appropriate recommendations regarding treatment alternatives or additional interventions that maximize patient outcomes taking into consideration choice of therapy, safety, efficacy, formulary status, and pharmaco-economics.

The PCPS will serve as a resource for advanced drug information support for the Pharmacy and Therapeutics (P&T) Committee and other staff. The PCPS with expertise in inpatient care will provide drug therapy management services through the establishment of a service aimed at reducing hospital readmissions and/or antibiotic stewardship. The PCPS will collaborate with other clinical specialists on pharmacy training to promote learning and performance of pharmacists, technicians, residents, and students. The PCPS will serve as a clinical instructor and preceptor for pharmacy students and residents.

The PCPS will maintain effective interdepartmental relationships by committee membership, problem resolution, and close cooperation with other departments to accomplish the organization\'s mission and goals. The goal is to foster a patient-focused, team-based multidisciplinary model of care that promotes rationale drug prescribing and monitoring to ensure positive outcomes are achieved with medication use.

Problem solving, team building, communication, and administrative skills are essential. This position requires a high degree of visibility and interaction with medical leadership.

Major Duties:

Approximately 50% of time is devoted to population-focused activities:

  • Pharmacy and Therapeutics Committee consulting/consultant
  • Drug use policy development
  • Formulary design
  • Development of clinical practice guidelines
  • Utilization review and monitoring of prescribing trends
  • Education for pharmacy staff and health care staff
  • Preceptor for residency and student rotations
  • Participation in performance improvement programs
  • Provide pharmacoeconomic consultations
Approximately 50% of time is devoted to patient focused activities:
  • Drug therapy management via antibiotic stewardship, service aimed at reduction in hospital readmissions, and/or patient care rounds
  • Drug information
  • Provider consultation and drug regimen reviews
  • Non-formulary consultations
  • Patient education
EQUIPMENT:

Standard office and pharmaceutical equipment.

WORK ENVIRONMENT:

Standard office environment. May be located wtihin the confines of a prison, jail or juvenile detention center. Security clearance is required; pre-employment drug testing is also required.

REQUIRED EDUCATION/EXPERIENCE:

Doctorate degree in Pharmacy from an accredited College of Pharmacy and ASHP-accredited PGY1 residency training. Current license or eligible to practice pharmacy in the state of Texas.

PREFERRED EDUCATION/EXPERIENCE:

PGY2 or Board Certified Pharmacotherapy Specialist (BCPS).

This allows pharmacists to perform in-depth profile review and to provide other services that support the health care staff in ensuring safe and effective use of medications. Clinical pharmacy services are ...decentralized. With a unique knowledge of medications, current research, and hands-on clinical experience, pharmacists develop solutions for managing pharmaceutical care. Services are designed to improve clinical outcomes and cost effectiveness. A range of services are offered and include medication therapy management, academic detailing and educational programs, drug information, P&T Committee consulting, non-formulary consulting, formulary management, and clinical guideline development. The Department serves as a clinical internship site for pharmacy students and offers an ASHP accredited residency program. The staff includes 9 managers, 29 pharmacists, 8 clinical pharmacists, 1 resident and 87 support staff.
