  • Perform in-person annual preventative health risk assessment for members, in the member\'s personal living space, running your assignments as your own mini-practice.

  • Take advantage of robust and supportive paid training, including in-person and online modules, as well as ongoing coaching sessions and team-building meetings.
  • Enjoy strong support from the management team; we view this as a critical role and take a proactive approach to providing the resources you need. For example, managers hold weekly office hours to respond to questions from IHA Nurse Practitioners.
  • Earn supplemental include that includes a flat rate per completed assessment, mileage reimbursement, potential performance bonuses, and an optional 401(k) plan.
Reporting to a Clinical Service Manager, you will perform an in-person, no-cost annual preventative health risk assessment for Medicare, Medicaid, and Affordable Care Act members of specific health plans. These assessments will take place within the member\'s personal living space according to market health plan expectations.

We have multiple opportunities for IHA Nurse Practitioners in Arizona, Maine, New Hampshire and New York, so feel free to share this information with your network. To meet the basic qualification for this role, you will have legal authorization to work permanently in the United States for any employer without requiring a visa transfer or visa sponsorship. In addition, to be a good fit for the Nurse Practitioner, IHA opportunity, you will have:
  • A master\'s or doctorate degree from an accredited NP program, or a master\'s degree from an accredited PA program.
  • Current RN and NP or PA licensure in the present state of practice.
  • Board certification by the American Nurses Credentialing Center (ANCC), the American Association of Nurse Practitioners (AANP), or the National Commission on Certification of Physician Assistants (NCCPA).
  • Current BLS certification.
  • Advanced understanding of risk adjustment models for the Center for Medicare and Medicaid Services (CMS) and the Affordable Care Act (ACA).
  • The ability to drive up to a 50-mile radius from your home address to complete appointments, a valid state driver\'s license and proof of sufficient insurance.
