The Registered Nurse will provide excellent patient care and will implement and maintain processes which will meet both personal and business goals of servicing our current patients. He/she will work within support teams for the achievement of patient satisfaction, company profitability, and long-term goals in line with company vision and values.
Duties/Responsibilities:
Represent our company well, both on and off the job.
Help with and oversee PCC with coordination of care activities to include:
Arranging deliveries of infusion/enteral patients.
Maintenance of infusion calendar.
Coordination of referrals from start to finish.
Assessment of the need for clinician input.
Tracking nursing visits, payments and nursing notes for infusion patients and contracted home health.
Enter patient information into HCN and run prescription labels and delivery tickets.
Help pack supplies and document NDC, lot numbers on appropriate paperwork. Document sharps container info in appropriate place.
Assist in ordering supplies, medications and enteral products needed.
Communicate with pharmacy tech regarding the days deliveries and any changes that were made, including the number of doses needed, what supplies needed and any lab work that may be drawn.
Communicate on a regular basis with home health agencies regarding new orders, labs and need for further or additional care.
Admit patients to services to include assessment of patient and instruction on Vital Care paperwork. This includes assessment of how patients will administer meds and if home health assistance is needed, what supplies will be best for patient, when labs will be drawn and very thorough explanation of how medication administration process works along with CR-BSI prevention and teaching patient to be advocate for their own care. Thorough instructions also include line care and when to contact VC for problems. Admit with knowledge that discharge planning begins on admission. Nurse to act as liaison between patient and MD and maintain constant contact with MD office.
Assist with maintain infusion and enteral pump tracking according to manufacturer's guidelines and ACHC standards, including yearly maintenance and servicing.
Maintain infusion suites to include caring for patients, scheduling patients, cleaning of rooms, etc.
Maintain each suite as required for ACHC.
Incorporate into EMP and Fire Plan the way patients will be handled in case of an emergency.
PICC lines and ordering of supplies needed.
Keep up with data tracking of PICCs.
Certified Infusion Instructor for Vital Care Inc. This includes teaching CE courses for home health agencies and hospitals.
Required Skills/Abilities:
Good interactive skills with employees, referral sources, physicians and consumers.
Education and Experience:
Registered Nurse (RN) licensure in good standing with the State Board of Nursing.
A minimum of three (3) years clinical experience.
a minimum of one (1) year experience in acute care or alternate site IV therapy.
Valid driver's license with good driving record.
Preferred, IV certification (i.e., CRNI) or other infusion-related certification.
