Responsibilities



Assist clients with daily activities such as bathing, dressing, grooming, mobility, and meal preparation to promote independence and dignity. Monitor and support medication management according to care plans, ensuring timely and accurate administration. Provide companionship and emotional support to reduce feelings of loneliness and enhance mental well-being. Perform light housekeeping tasks including laundry, cleaning, and organizing to maintain a safe and comfortable living environment. Observe and report any changes in the client's health or behavior to supervisors or healthcare professionals promptly. Support clients with transportation to appointments, errands, or social activities as needed. Follow established care protocols while maintaining strict confidentiality and respecting client preferences. Collaborate with family members and healthcare teams to ensure comprehensive, coordinated care tailored to each individual's needs.

Qualifications



Prior experience in home health care or related fields is preferred; experience with diverse populations including geriatrics is highly valued. Knowledge of medical-surgical procedures, post-acute care practices, or outpatient clinic environments enhances your ability to deliver quality care. Experience working with clients in settings such as Level I Trauma Centers or teaching hospitals demonstrates adaptability and clinical competence. Familiarity with emergency response procedures and trauma-informed care is advantageous for handling urgent situations effectively. Strong communication skills, patience, empathy, and a proactive attitude are essential for building trust with clients. Ability to perform physical tasks such as lifting or assisting with mobility safely; valid driver's license preferred for transportation duties.
