We are seeking an In Home Caregiver to join our team in Cape May, NJ. This role is crucial in delivering personalized care and support to our clients, ensuring their comfort and well-being at home. As an In Home Caregiver, you will play a vital part in our mission to provide exceptional health services.

Responsibilities

Provide compassionate personal care and assistance with daily living activities. Monitor client health and report any changes to the healthcare team. Assist with medication management as directed by healthcare professionals. Ensure a safe and clean environment for clients. Engage clients in meaningful activities to promote mental and emotional well-being. Collaborate with family members and healthcare providers to develop care plans. Maintain accurate records of client care and progress. Uphold the values of Heart to Heart in all interactions.

Qualifications:

Strong communication skills and ability to build rapport with clients. Knowledge of medical-surgical practices is preferred. Familiarity with post-acute care processes is advantageous. Ability to work independently as well as part of a team. Compassionate demeanor with a commitment to client-centered care.
