Responsibilities



Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support Monitor patients' health by checking vital signs and observing for any changes in condition Administer medication as prescribed, following HIPAA guidelines to ensure privacy and confidentiality Prepare nutritious meals tailored to individual dietary needs and assist with meal planning and cooking Support clients with mobility using equipment like Hoyer lifts or transfer belts to ensure safe transfers Maintain accurate documentation of care plans, patient observations, and progress notes Perform light housekeeping duties including cleaning, laundry, and maintaining a safe environment Provide emotional support and companionship to reduce feelings of loneliness or isolation Follow all safety protocols including first aid procedures and CPR when necessary Collaborate with healthcare professionals to implement care plans effectively Assist clients with transportation needs for appointments or errands within the community Support individuals with developmental disabilities, Alzheimer's disease, dementia, autism spectrum disorder or hospice needs as required

Skills



Proven experience in home health care, assisted living, nursing homes, or group homes is highly preferred Knowledge of patient observation techniques and ability to monitor vital signs accurately Familiarity with HIPAA regulations ensuring client privacy at all times Ability to perform heavy lifting safely when required for patient transfers or mobility assistance Strong skills in medication administration and understanding of care plans Experience working with people with disabilities or developmental challenges such as autism or dementia is advantageous Excellent communication skills to interact effectively with clients and team members Basic knowledge of first aid, CPR certification preferred but not mandatory; training provided if needed Ability to work independently while following detailed instructions from healthcare professionals Compassionate attitude combined with patience and resilience in challenging situations
