We are seeking an experienced Interdisciplinary (IDT) Coordinator to join our team!

We are actively recruiting for an IDT Coordinator to work with our PACE team. This position requires a dynamic individual, who enjoys working in an environment that encourages developing relationships with our participants and their caregivers, where a team focused approach to care is the cornerstone of our organization.

Specifications

Education: High School Diploma or GED equivalent required; Related Associates Degree preferred
Experience: Minimum two (2) years of related clerical or secretarial experience required; PACE experience preferreD.

Number/Type of Employees Supervised: None

Licensure, Registry, or Certification Required: Valid MI driver\'s license preferred.

Special Training: Only acts within scope of practice. Meet a standardized set of orientation and/or individual competencies for the specific position description established by PACE of Southwest Michigan, and approved by CMS before working independently.

Immunizations: Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact.

Specific Requirements and Considerations: May be subject to exposure and handling of infectious waste, diseases, conditions or hazardous chemicals, etc., including potential exposure to TB, AIDS, and/or Hepatitis B.

Ages of Patients Rendered Care: Adult, Geriatric

Key Responsibilities

\xc2\xb7 Types letters, correspondence, forms, reports, meeting minutes and other documents which are often confidential in nature

\xc2\xb7 Attends and serves as administrative support during IDT documenting minutes

\xc2\xb7 Generates letters and documents in participants\' medical records

\xc2\xb7 Coordinates care conferences, maintains agendas for morning huddles and IDTs, documents meeting minutes and manages meeting follow-ups

\xc2\xb7 Coordinates process for denial and reduction of service letters

\xc2\xb7 Schedules IDT assessment for participant\'s reassessment and annual assessments

\xc2\xb7 Coordinates Medicaid redetermination process and works with DHHS Representative and Center Manager to ensure completion and compliance

\xc2\xb7 Interacts with all levels of staff, participants, families and the general public on the phone and in person, offering information and assistance or directing the person to the appropriate department

\xc2\xb7 Attends staff and committee meetings as directed

\xc2\xb7 Serves as office receptionist (back-up) as directed

\xc2\xb7 Assists in the SDR process

\xc2\xb7 Abides by all established PACE of Southwest Michigan policies, rules and regulations

\xc2\xb7 All other duties as assigned

Skills and Knowledge

\xc2\xb7 Computer skills necessary to effectively utilize word processing and spreadsheet software applications

\xc2\xb7 Proficient typing and word processing skills necessary for the creation and compilation of various documents, communications and reports

\xc2\xb7 Interpersonal skills necessary to communicate effectively with all levels of staff, participants, families and the general public in the exchange of information and to handle problem situations in a courteous, tactful and respectful facility

\xc2\xb7 Must be able to multi-task and work independently and in a team setting

\xc2\xb7 Effective verbal and written communication skills

\xc2\xb7 Excellent organizational skills and attention to detail

\xc2\xb7 Able to work with the Interdisciplinary Team (IDT) setting
