The Housing Coordinator serves as part of the Assertive Community Treatment (ACT) team and is responsible for providing housing assistance for clients and managing the Supported Apartment Program (SAP) to coordinate support and services to clients/tenants in the program. This position will function as the Housing Coordinator for those with a serious mental illness (SMI) diagnosis receiving ACT and/or SAP housing services. This role facilitates access to appropriate housing supports and services within SMI programs and approaches all clients with a whole health perspective, identifying all social determinant needs that impact housing and overall client wellbeing. The Housing Coordinator will possess a demonstrated professional history in networking and collaborating with community partners and accessing community housing resources. This position must possess strong skill in working with individuals with SMI diagnoses, show an ability to work independently, have a history of progressive career responsibilities, and possess professional oral and written communication styles.The Housing Coordinator is part of the integrated care system and participates as part of the care team, identifying ways to improve the program. This position will monitor success, and strengthen access, stabilization, and progress to help individuals reach higher levels of housing independence. The Housing Coordinator is a positive team player that promotes a culture of inclusion and participation.
Required Education and Experience
Bachelor's degree in psychology, social work or other behavioral health related field
One year of full-time employment experience working directly with adults in a human service setting post-degree. A master's degree in psychology, social work or other behavioral health related field may be substituted for this experience
Must complete a case management or service coordinator certification program offered and approved by the state with six months of employment
Valid driver's license, reliable operating transportation, and proof of current auto insurance
Preferred Education and Experience
Master's degree
Knowledge of mental health issues, community supports and ability to work well in a team environment
Knowledge of Recovery based interventions in Case Management services
Essential Functions

Create a collaborative environment that promotes positive relationships with key partners including DCBS, Lexington Housing Authority, Guardianship, property management, courts, law enforcement, mental health advocacy groups, and other community networks
Operate as an ambassador of New Vista, connecting community partners and clients to services within the organization (e.g., crisis, SUD, IDD, SMI, primary care and housing and employment services)
Serve as an expert collaborator on housing resources and community partnerships for individuals with SMI and housing needs through attending local community partner meetings, establishing resources, bolstering landlord relationships, and other functions that support clients with SMI and address barriers to successful housing
Communicate and coordinate with multiple community partners, acting as the face of New Vista to housing authorities, property management, guardians (when appropriate) and other key partners
Monitor client engagement in clinical services, reporting key metrics to Regional Director of Specialty and Outreach Services, and lead innovative approaches to maximize client engagement
Work with leadership to oversee SAP program finances, address sustainability and determine innovative approaches for SAP and ACT participants to utilize financial resources such as CoC permanent supportive housing programs and housing vouchers to help sustain housing
Problem-solve with a level of independence for issues including but not limited to client functioning, needed resources, property management concerns, and community partner inquiries
Meet with clinical teams, doctors, family members, and guardians to discuss client/tenant functioning
Attend staff meetings and supervisions, assessing needs, risk and opportunities for each client
Support medication acquisition and treatment engagement through assisting with establishing supports that keep clients housed and stable (e.g., connecting to delivery prescriptions, appointment reminders and assessing and facilitating access to transportation and telehealth, etc.)
Interact regularly with clients/tenants to determine needs and assist with making progress in achieving mental wellbeing, socialization skills and independent living goals
Collaborate with team to help ensure client access to appropriate housing, identifying supports and ways to maintain an individual in a stable home environment
Assist client/tenants with finances/budget
Provide transportation to outings, grocery shopping, and appointments
Coordinate support service activities to include assisting with scheduled appointments, referrals to appropriate New Vista services, providing links to community resources, motivating and preparing clients/tenants in their transition into independent living
Coordinate referral screenings, admissions, and discharges
Case Management/Rehabilitation/Psychosocial Care
Provide direct face-to-face services to promote socialization, meaningful daily activity and recovery
Provide psychoeducation relating to health management, natural supports and individual strengths
Assist clients in development of their recovery plan, including psychiatric advance directives and crisis plan
Implement and coordinate recovery plans for individual clients as assigned, including evaluation, consultation, education, and follow-up services
