Overview


We are seeking a compassionate and dedicated

Social Worker

to join our healthcare team. The ideal candidate will provide comprehensive support to patients and their families across various settings, including hospitals, outpatient clinics, nursing homes, and community-based programs. This role involves delivering clinical counseling, case management, and crisis intervention services to promote mental health, behavioral well-being, and overall quality of life. Experience with diverse populations such as children, seniors, individuals with disabilities, or those requiring specialized care like dementia or PTSD is highly valued. The Social Worker will ensure compliance with HIPAA regulations and maintain accurate medical documentation using EMR systems.

Duties



Conduct patient assessments to determine needs and develop individualized care plans Provide clinical counseling including behavioral therapy, motivational interviewing, and psychotherapy for individuals and groups Assist with intake processes and coordinate services for patients in inpatient, outpatient, hospice, or post-acute care settings Manage case loads involving patients with developmental disabilities, autism spectrum disorders, dementia, or mental health conditions such as PTSD or addiction Collaborate with multidisciplinary teams to develop treatment strategies and monitor patient progress Collect and document medical records accurately while adhering to HIPAA guidelines Perform crisis intervention and manage emergency situations effectively Support families through counseling sessions focused on child & family therapy or hospice care needs Utilize ICD-9/10 coding and CPT coding for documentation purposes when applicable Engage in clinical research projects related to behavioral health or social work practices Maintain proficiency in medical terminology and EMR systems for efficient record keeping

Experience



Proven experience in social work within healthcare environments such as hospitals, nursing homes, assisted living facilities, or outpatient clinics Familiarity with medical documentation standards and HIPAA compliance Experience working with diverse populations including children, seniors, individuals with disabilities or developmental disorders, and those requiring dementia or PTSD care Background in crisis management, behavioral health interventions, addiction counseling, or special education is preferred Knowledge of ICD coding systems (ICD-9/10) and CPT coding is advantageous Demonstrated ability to perform diagnostic evaluations and conduct individual/group therapy sessions Experience working with EMR systems for documentation and data collection purposes

Pay: From $44,000.00 per year
