Job Description

Three Rivers Hospice is actively seeking an RN for management and coordination of their office located in Cassville, Missouri. The role of the Hospice RN Coordinator is to provide leadership and direction in accordance with organizational and departmental goals and objectives and to provide for professional growth and development of staff through identification of needs and encouraging educational efforts. The individual for this position will also be responsible for seeing all information on charts, care plans, medications and treatment sheets are updated and correct. Hospice RN Coordinators work with all members of the interdisciplinary teams and attend all Interdisciplinary Care Plan meetings every two weeks. The Hospice Coordinator plays a vital role in ensuring that every patient's final journey is met with dignity, comfort, and respect--while supporting families through one of life's most challenging transitions.

Key Responsibilities:

Care Coordination:

Schedule and organize patient visits, ensuring seamless communication between nurses, physicians, social workers, chaplains, and volunteers.

Family Support:

Serve as a compassionate liaison for patients and their loved ones, answering questions, providing resources, and offering emotional reassurance.

Administrative Duties:

Maintain accurate patient records, manage referrals, and ensure compliance with hospice regulations and policies.

Pay: $75,000.00 - $80,000.00 per year
