Position Overview


The

Hospice Registered Nurse

is responsible for pain management, symptom control in the delivery of care to Hospice patients, as established by the plan of care, the interdisciplinary team, attending physician and Hospice Medical Director.

Part-Time weekly visits needed Monday - Friday for Sumner, Smith, Macon, Wilson, and Davison (only Donelson and Hermitage).

Essential Job Functions:

Develop a plan of care through physician orders, client input, and hospice team. Educate clients and their family members based on client's specific needs. Properly orient, train and supervise primary caregivers to ensure the most optimal functioning level for each client. Coordinate the continuum of hands on client care and collaborate with the interdisciplinary team through documentation and timely communication with the client's physician and other caregivers. Provide care in patients home or facility or residence using a variety of skills to assess, identify problems, create nursing interventions, goals, education, and outcomes of interventions

Requirements:

An active RN License in the state of application Valid CPR Valid Drivers License and Auto Insurance

Preferred:

1-year RN experience in a hospice setting Home Care Home Base experience a plus
