Responsibilities:
Provide compassionate and quality care to hospice patients and their families
Conduct comprehensive assessments of patients' physical, emotional, and psychosocial needs
Develop and implement individualized care plans in collaboration with the interdisciplinary team
Administer medications and treatments as prescribed by the physician
Monitor and evaluate patient's response to treatment and adjust care plan as needed
Educate patients and families on disease process, symptom management, and end-of-life care options
Provide emotional support and counseling to patients and families
Coordinate with other healthcare professionals to ensure continuity of care
Document patient care activities accurately and timely
Qualifications:
Valid RN license in the state of Illinois
Minimum of 1 year of nursing experience, preferably in hospice or palliative care
Knowledge of infection control principles and practices
Familiarity with sterile processing techniques
Experience in family planning and counseling
Ability to lift heavy objects and assist with ambulatory patients
Familiarity with dementia care techniques
Knowledge of medical imaging procedures and interpretation
Experience with esthetic laser treatment is a plus
Familiarity with durable medical equipment (DME) usage and maintenance
Proficiency in sonography is a plus
