We are seeking a compassionate and highly skilled Hospice Nurse to provide exceptional end-of-life care to patients in a home or facility setting. The ideal candidate will have extensive experience in palliative and hospice care, with a strong background in managing complex medical conditions, medication administration, and patient assessment. This role requires a dedicated professional capable of working independently while collaborating with interdisciplinary teams to ensure comfort, dignity, and quality of life for patients facing serious illnesses. Knowledge of EMR/EHR systems, HIPAA compliance, and advanced nursing procedures is essential. The Hospice Nurse will play a vital role in delivering holistic care tailored to the unique needs of each patient and their families.
Responsibilities
Conduct comprehensive patient assessments including vital signs, physical examinations, and symptom management.
Develop and implement individualized care plans aligned with hospice standards and patient wishes.
Administer medications including injections, IV infusions, and pain management therapies such as opioid titration and acute pain control.
Manage complex medical equipment such as ventilators, catheters, ostomy devices, and suctioning systems.
Provide wound care, ostomy care, and perform sterile procedures including suturing when necessary.
Educate patients and families on disease progression, medication administration, symptom management, and end-of-life planning.
Coordinate with physicians, social workers, chaplains, and other healthcare professionals to ensure comprehensive palliative support.
Document all patient interactions accurately within EMR/EHR systems following HIPAA regulations.
Assist with discharge planning and facilitate transitions between care settings when appropriate.
Maintain infection control protocols and aseptic techniques during all procedures.
Respond promptly to urgent symptoms or crises with appropriate interventions including airway management or ventilator support if needed.
Experience
Proven experience in hospice & palliative medicine or related fields such as critical care, ICU, emergency medicine, or long-term care settings.
Familiarity with advanced nursing procedures including IV insertion, medication administration via multiple routes (including IV infusion), tube feeding management, and catheter care.
Knowledge of ICD-10/ICD-9 coding for documentation purposes.
Experience working with diverse populations such as geriatrics, pediatrics (including neonatal), individuals with developmental disabilities or dementia care needs.
Proficiency in EMR/EHR systems like Epic, Cerner, Athenahealth, or eClinicalWorks is preferred.
Strong understanding of medical terminology, physiology knowledge, diagnostic evaluation techniques, and infection control practices.
Additional skills in behavioral health support, pain management strategies, discharge planning, trauma medicine (Level I/II trauma center experience), post-acute care experience, or home & community care are advantageous.
