What You'll Do

As a

Hospice Nurse Practitioner (F2F)

at LifeChoice, you will:

Perform

face-to-face visits

for hospice recertification to determine continued eligibility per Medicare guidelines. Complete comprehensive patient assessments--physical, emotional, and psychosocial--typically in homes, assisted living, or skilled nursing facilities. Collaborate with the hospice Medical Director and interdisciplinary team to determine prognosis, recertification, and ongoing plan of care. Document visits thoroughly and compliantly to support hospice eligibility and regulatory requirements. Educate patients and families on hospice goals, symptom management, and changes in condition with empathy and clinical confidence. Serve as a resource to the clinical care team, providing guidance on best practices and quality standards. Help uphold LifeChoice's commitment to compassionate, ethical care through every interaction.

What You Bring

Active and unrestricted

Nurse Practitioner license

in the state of Illinois Board certification (FNP, AGNP, or related discipline) Valid

driver's license

and access to a reliable vehicle

At least 1 year of experience in hospice, palliative care, geriatrics, or home health

(strongly preferred) Solid understanding of

Medicare hospice guidelines

and compliance requirements Ability to work independently and navigate diverse patient environments Strong documentation skills and attention to detail (bonus if you're EMR-savvy) Empathetic communication skills--you're as great with families as you are with your clinical team
