Job Description

We seek a Registered Nurse (RN), preferably with Hospice, Palliative Care, Home Health, or Med Surg experience, to be part of the team at a highly respected Hospice and Palliative Care group located on Martha\'s Vineyard.
Responsibilities include:
- Assume responsibility for coordinating patient care for assigned caseload
- Complete initial and ongoing assessments of patient and family to determine hospice needs.
- Initiate POC and maintain/update POC as needed
- Prescribe appropriate rehab and preventive care as needed
- Assist patient with daily living activities
- Participates in (paid) on-call duties
- A Massachusetts RN license in good standing
- The desire to be part of the community
- Ability to communicate clearly and effectively
