Job Summary

We are seeking a compassionate and dedicated Medical Social Worker to join our team. The ideal candidate will provide essential support to patients and their families, helping them navigate the complexities of healthcare systems and access necessary resources. This role involves working closely with individuals in assisted living facilities, home community care settings, and clinical environments to ensure holistic care is provided.

Duties

Conduct assessments to determine patients' social, emotional, and environmental needs. Provide individual and group counseling to patients and families facing health-related challenges. Develop and implement crisis management strategies for patients in distress. Collaborate with healthcare professionals to create comprehensive care plans that address both medical and social needs. Maintain accurate medical records and documentation of patient interactions and progress. Advocate for patients' rights and access to services within the healthcare system. Facilitate communication between patients, families, and healthcare providers to ensure continuity of care. Offer support in special education settings for children with medical or social challenges.

Requirements

Master's degree in Social Work (MSW) from an accredited institution. Valid state licensure as a clinical social worker or eligibility for licensure. Experience in assisted living, home community care, or clinical counseling preferred. Strong understanding of crisis management techniques and systems within healthcare. Proficient in maintaining medical records with attention to detail. Excellent interpersonal skills with the ability to engage effectively with diverse populations. Knowledge of special education resources is a plus. Ability to work collaboratively within a multidisciplinary team environment.
