Hospice Registered Nurse - Evenings

4p-8a

Weekend Requirement:

Every other weekend and every other holiday

Position Summary

Applies the nursing process with patients and families, integrating principles that reflect sensitivity to their age-specific/developmental needs, gender, culture, religion, psychosocial and socioeconomic backgrounds in order to provide evidence-based, holistic care in a manner that respects the worth and dignity of each individual. Practices professional nursing as a registered nurse within the legal and ethical framework established by the Connecticut Nurse Practice and Code for Nurses, American Nurses Association Home Health Nursing, National Hospice Association and applicable regulatory standards. Responsible for the coordination of all aspects of Hospice patient care including coordination of referrals and patient care related supervisory functions, and collaboration with Hospice Interdisciplinary team.



This RN covers the evening and overnight hours for hospice homecare, triaging calls, making home visits and pronouncements as needed to hospice and palliative care patients to ensure care is available 24 hours a day.

Minimum Qualifications:

Current RN license state of CT


BSN from accredited nursing school


1 year clinical experience

Preferred Qualifications:

Experience in home health and/or hospice and palliative care.
