As a
Hospice Coordinator for Spiritual Care/Bereavement/Volunteer
, you will:Ensures that appropriate spiritual services are provided in a timely manner by either providing such to the patient/family or coordinating the provision of services with community clergy or spiritual counselors.
Provides direct spiritual services to patients/families according to their belief system and practice.
Maintains records of spiritual services utilization and related activities as directed for quality assessment and performance improvement (QAPI), hospice program development, and policies and procedures review and revision.
Acts as the hospice spiritual services community liaison by developing community contacts and offering hospice education to a variety of clergy, counselors and congregations.
Receives referral calls, completes intakes, documents new client services enrollment, and schedules care professionals for hospice
Update/upload documents to patient charts
Assess bereavement risk for patients' families and caregivers Develop and implement individualized bereavement plans
Provide grief counseling, education, and support groups
Maintain bereavement services for families for up to 13 months following death
Ensure compliance with hospice and regulatory bereavement standards
Recruit, train, schedule, and supervise hospice volunteers
Match volunteers with patients and families based on needs and skills
Maintain volunteer documentation and compliance requirements
Support and recognize volunteers to promote engagement and retention
To qualify for a
Hospice Coordinator for Spiritual Care/Bereavement/Volunteer
with us:
Masters or Doctoral preparation in social work, psychology, counseling, or equivalent theological education
One (1) year experience in the past three (3) years in individual, family or group bereavement counseling
One (1) year appropriate supervisory or program coordination experience that is relevant to the supervision of bereavement volunteers/coordinators
Proof of driver's license and auto insurance required.
