Position Summary
Trek Healthcare is seeking a compassionate and mission-driven Hospice Chaplain to join our interdisciplinary hospice team in the Butte area. The Hospice Chaplain provides spiritual, emotional, and psychosocial support to patients and families facing serious illness, death, and bereavement, in accordance with patient beliefs, values, and preferences. This role is an essential member of the hospice Interdisciplinary Group (IDG) and supports holistic, patient-centered care consistent with CMS and ACHC hospice standards.
Essential Duties and Responsibilities
The Hospice Chaplain provides spiritual assessment, counseling, and support to hospice patients and their families across care settings, including private homes, assisted living facilities, and skilled nursing facilities. Services are provided in a non-denominational, inclusive manner that respects cultural, spiritual, and religious diversity. The Chaplain collaborates closely with nursing, social work, medical providers, volunteers, and other members of the interdisciplinary team to support the patient's goals of care. Participation in IDG meetings is required, including contributing to the development, implementation, and ongoing evaluation of individualized hospice care plans. The Chaplain provides emotional and spiritual support during end-of-life transitions, crisis situations, and actively dying phases, and may assist with funeral planning, rituals, or coordination with community clergy when requested. Bereavement and grief support for patients, families, and caregivers is provided as appropriate and in coordination with the hospice bereavement program. Accurate, timely documentation of all visits and interventions is completed in the electronic medical record in compliance with CMS and ACHC standards.
Required Qualifications
A combination of education and experience will be considered. A bachelor's degree in theology, divinity, religious studies, pastoral care, counseling, or a related field is a plus. Formal training in spiritual care, pastoral counseling, or hospice chaplaincy is required. Experience providing spiritual care in hospice, palliative care, healthcare, or a related setting is strongly preferred. The candidate must demonstrate the ability to provide spiritual care to individuals of all faiths, belief systems, and those who identify as non-religious. Strong communication, documentation, and interpersonal skills are essential. A valid Montana driver's license, reliable transportation, and current auto insurance are required. Candidates must be able to pass background screening and meet all hospice employment requirements.
Preferred Qualifications
Completion of Clinical Pastoral Education (CPE) units is preferred. Ordination, commissioning, or endorsement by a recognized faith community is preferred but not required. Experience working as part of an interdisciplinary healthcare team and familiarity with hospice philosophy and end-of-life care is highly desirable.
Physical and Travel Requirements
This position requires the ability to travel within the Butte service area, enter patient homes and facilities, and provide care in various environments. The role may involve periods of standing, walking, and sitting, as well as emotional resilience when supporting patients and families at end of life.
