As a Navian Hawaii hospice team member, you will guide patients and their family on their journey, delivering comfort and support with a caring heart, replacing fear with hope, helping them find the beauty and joy in every moment, and providing each person with the kindness and dignity they deserve.

POSITION SUMMARY

The Chaplain is responsible for working in collaboration with the patient, primary care physician, medical director, clinical nursing leadership and other palliative care team members in an active practice to deliver spiritual and psych-social services to patients and families.

Primary Responsibilities:

  • Provides direct spiritual care to patients and families.
  • Assures spiritual assessment of patients in the hospice program and appropriate services are rendered in a timely manner.
  • Participates in the interdisciplinary team and contributes to the plan of care.
  • Maintains records of spiritual care services utilization and related activities for quality assurance, program development, and policies and procedures review and revision.
  • Provides consultation, education, and support to the interdisciplinary team on spiritual care.
  • Actively participates in quality assessment performance improvement teams and activities as needed.
  • Adheres to the Association of Professional Chaplains Common Code of Ethics, which guides decision making and professional behavior
  • Respects the confidentiality of information from all sources, including the patient, medical record, other team members, and family members in accordance with federal and state law, regulations, and rules.
  • Models and collaborates with the organization and its interdisciplinary team in respecting and providing culturally competent patient centered care.
  • Seeks and creates opportunities to enhance the quality of spiritual care practice
Additional Responsibilities:
  • Collaborates with local clergy and other spiritual care leaders in augmenting patient\'s faith group support
  • Maintains an active professional relationship and maintains good standing with one\'s faith/denomination.
  • Adheres to and exhibits Navian Hawaii core values
  • Adheres to organizational specific safety standards and guidelines
  • Demonstrates exceptional customer service and interacts effectively the interdisciplinary team.
  • Consistently represents Navian Hawaii to all internal and external customers with integrity and professionalism.
Knowledge and Skills Requirements:
  • Knowledge of and commitment to hospice philosophy of care.
  • Ability to work as a member of interdisciplinary team/group.
  • Comfortable to work in an interfaith setting.
  • Ability to network with community clergy and congregations.
  • Ability to accept different lifestyles, cultures, beliefs, and values.
  • Ability to communicate in verbal and written in English
  • Ability to work harmoniously with a broad range of staff, volunteers, and the community as a representative of Hospice Hawai\'i, Inc.
  • Demonstrates good judgment in seeking and accepting guidance, facilitates clear and honest communication with other team members.
  • Consistently demonstrates ethical conduct and judgment that is in harmony with the mission and values of Navian Hawaii.
Experience and Educational Requirements:
  • Masters of Divinity Degree or its equivalent
  • Four (4) units of Clinical Pastoral Education or equivalent
  • Ordination or certification from the candidate\'s faith group
  • Certified or eligible to be certified by one of the recognized chaplain\'s associations
  • Experience working with death and dying, individuals/families/caregivers.
Equipment Used
  • Operates phone system, computer (EMR [electronic medical record], Word, Excel, Microsoft Outlook, and Email), copier, etc.
  • Medical equipment - hospital bed, wheelchair, walker, commode, shower chair, oxygen tanks and lifts.
  • Frequent use of own personal motor vehicle which meets valid registration and insurance requirements.
Physical, Mental and Communication Demands
  • Requires use of hands and must be able to speak, read and write English. Vision abilities include close vision, depth perception, and ability to focus. Must be able to hear clearly.
  • Requires frequent standing, walking and sitting, with occasional bending, squatting and kneeling. Ability to lift and move up to 25 lbs unassisted.
  • Occasionally requires long periods of sitting/sitting while driving.
  • Must be flexible and able to work on multiple tasks simultaneously and meet multiple deadlines as well as analyze problems and adopt an effective course of action. Requires learning a number of policies and procedures, writing, arithmetic and problem solving skills.
  • In addition to being able to fulfill the physical, mental, and communication demands listed above, must be able to follow safety instructions and demonstrate critical thinking.
Employment Requirements:

All offers are contingent on a candidate\'s successful completion of the following requirements:
  • A legally compliant background check
  • Drug Screen & Physical Exam
  • Current TB clearance
  • CPR certification
  • Use of personal motor vehicle which meets valid registration and insurance requirements. Must possess a valid Drivers\' License, current auto insurance, and reliable transportation.
  • Must have acceptable driver\'s abstract.
Hours:
  • Generally, Monday through Friday, 8:00 am through 4:30 pm. Requires intermittent On-Call supervisory coverage
