Summary
We are seeking a Registered Nurse Case Manager to join our compassionate team at Wren Hospice for the Upstate area. Here at Wren Hospice, we transform lives daily as we work for an organization that truly cares about making every moment count. The registered nurse plans, organizes, directs hospice care and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities.
