About the Role
We're seeking a compassionate and skilled RN Hospice Case Manager to join our hospice team delivering high-quality, patient-centered care in the community setting. In this role, you'll work independently in the field, bringing comfort and support to patients and their families as you assess, plan, coordinate, and evaluate hospice care -- ensuring dignity and quality of life for every patient.
What You'll Do
Patient Care & Clinical Responsibilities
Conduct comprehensive assessments and ongoing evaluations of patient condition and needs in home and facility settings.
Develop, implement, and revise individualized care plans based on clinical findings and hospice philosophy.
Provide patient and family education, including disease management, symptom control, and equipment guidance.
Support patients with high-acuity needs (e.g., drains, PCA pumps, IV therapy, wound care) with clinical knowledge and troubleshooting. (On-site training provided)* Collaborate with physicians, interdisciplinary team members, and support staff to ensure consistent, holistic care.
Document clinical assessments, care interventions, and outcomes accurately in the EMR (Epic experience preferred).
Caseload & Environment
Manage a field caseload of approximately 22-25 patients per week (~6 visits daily).
Provide continuity of care in patient homes, assisted living facilities, group homes, and nursing homes.
Work independently with strong organizational and communication skills.
Required Skills & Certifications
Registered Nurse (RN) License (state-appropriate).
Basic Life Support (BLS) certification required.
Strong patient/family education and clinical decision-making skills.
Excellent communication with providers and care teams.
Knowledge of medical equipment troubleshooting and hospice care principles.
EMR experience (Epic preferred).

Orientation & Tools
Up to 2 weeks of orientation based on your comfort level.
Company-provided laptop, cell phone, and PPE.
N95 fit testing required prior to start.

Pay: $3,200.00 - $3,400.00 per week
