• -Coordinate the development and implementation of a plan of care as desired by patients families

- Assists families in understanding the grief process.
- Develops, coordinates and evaluates all bereavement activities.
- Conduct or make arrangements for services to honor the deceased when requested
- Ensures all required Bereavement Mailings are sent on time either by the Bereavement Coordinator or by volunteers.
- Collaborate with the IDG team members to address the spiritual needs of patients and bereavement needs of patients families.
- Able to work independently and as a part of the multidisciplinary team.
- Ability as a Chaplain to provide spiritual care and guidance specific to the patients and caregivers needs and preference of religion.
-Coordinates with other Hospice Team members in creating at least one meaningful Memorial Service per calendar year. -Participates in ongoing quality assessment and performance improvement -Submits twelve hours of continuing education each calendar year related to the Hospice role

• Education desired:

- Preferred Bachelor\'s or Master\'s degree in Social Work, Psychology, Mental Health counseling, Pastoral Counseling or the documented equivalent in education, training or experience per state regulations. -Two units of Clinical Pastoral Education or Bachelor\xe2\x80\x99s degree in theology, divinity, pastoral care, philosophy, religion or related discipline.

• Experience Required: -A minimum of two years\xe2\x80\x99 experience in a health care setting and/or related pastoral experience with grief and loss counseling.
