As a Hospice Agency Coordinator, you will play a pivotal role in ensuring that our patients receive compassionate and comprehensive end-of-life care. You will be the driving force behind coordinating services, managing schedules, and facilitating communication among healthcare providers, patients, and their families. Your organizational skills and empathetic nature will help create a supportive environment for those in need during their most critical times.
What you'll do
Coordinate patient admissions and discharges to ensure seamless transitions in care
Collaborate with healthcare professionals to develop individualized care plans tailored to each patient's needs
Schedule and manage appointments for interdisciplinary team members, including nurses, social workers, and chaplains
Serve as a primary point of contact for patients and families, providing updates and addressing concerns with empathy and clarity
Maintain accurate patient records and documentation in compliance with regulatory standards
Facilitate communication between patients, families, and the hospice team to promote understanding of services offered
Assist in training new staff members on agency protocols and procedures
Participate in quality improvement initiatives to enhance patient care services
Basic qualifications
Strong organizational skills with the ability to manage multiple tasks efficiently
Excellent verbal and written communication skills for effective interaction with diverse populations
Preferred qualifications
Experience in healthcare coordination or hospice settings
Familiarity with electronic health record (EHR) systems
Knowledge of hospice regulations and compliance standards
