Responsibilities and Duties
Receive information from referral sources and coordinate care for patients
Verify and enter physician orders
Identify level of care needed
Receive and give verbal orders accurately
Exceptional interpersonal and customer service skills
Ability to communicate clearly with pharmacist
Work closely with referring hospitals, physicians, skilled nursing facilities, patients, and families
Manage after-hours and weekend referrals (rotating on call)
Qualifications:
Current license as a licensed LPN from the Utah State Board of Nursing
Good verbal and written communication skills
Basic computer knowledge
Ability to effectively problem solve
Provide excellent customer service
Knowledge of Medicare/Medicaid home care and hospice criteria preferred
Knowledge of Medicare, Medicaid and 10A criteria
