Job Description


Phoenix Home Care & Hospice is seeking a Home Health Intake and Scheduling Specialist.

The specialist will work closely with the Operation Teams to ensure patient profile, demographics and authorizations are obtained, maintained, and entered in our system with the highest quality in a timely manner.

Position Responsibilities:

  • Enter new patient profile and maintain demographics on existing patients in the database.
  • Coordinate the scheduling needs for current and upcoming patients
  • Handle incoming calls efficiently
  • Manage workflow of incoming referrals
  • Effectively communicate patient needs to Home Helth Teams
  • Verify all paperwork needed for setting up Home Health Services
  • Other duties as assigned
Requirements
  • Intake and scheduling experience preferred but not required
  • Excellent data entry skills
  • Must be self-motivated
  • Must have past customer service experience
  • Previous experience working in an office/professional setting.
  • Ability to work independently as well as in a team
  • Ability to multitask and stay organized
  • Be able to communicate effectively and professionally with internal and external customers
  • Previous experience working with Home Health is preferred but not required
  • Must have High School Diploma
  • Must have current driver\'s license
