Key Responsibilities:

Deliver care based on individualized plans created with the RN Case Manager Perform wound treatments and support clients with medication management Provide education to clients and family members on daily care routines Collaborate with the RN Case Manager, Clinical Supervisor, and Care Team to ensure high-quality service

What You'll Need to Succeed:

Current LPN license

in the State of Minnesota

Reliable vehicle

, valid driver's license, and auto insurance Ability to work independently, manage time well, and adapt to a changing environment Experience in

TCU, hospice, or home health is a plus

Familiarity with Electronic Medical Records (EMR) preferred
