Summary

Provides support services, under the supervision of the appropriate professional staff, to maintain, strengthen and safeguard the care of patients who have elected our Private Duty and or home health services.

Essential Job Functions/Responsibilities

1. Applies safety principles and proper body mechanics in the performance of specific techniques of personal and supportive care.

2. Recognizes emergency situations and follows appropriate emergency procedures when indicated.

3. Demonstrates basic infection control procedures and follows agency guidelines

especially with regard to handling of hazardous wastes.

4. Renders services in strict accordance with the written care plan.

5. Provides personal care service, such as

maintaining a clean, safe and healthy environment

bed, sponge, tub, or shower bathing

shampooing in sink, tub or shower

caring for nails and skin

oral hygiene

assisting with toileting and elimination

6. When necessary provides transportation to appointments and for family functions or outings.

7. Performs homemaking and other environmental services that facilitate sanitary and safe care at home. Routine housework including but not limited to: laundry, mopping, dusting.

8. Observes, reports and documents changes in patient status to the appropriate case manager.

9. Maintain records and keep activity records current, submits documentation according to agency guidelines.

10. Performs support services, such as meal preparation, laundry, grocery shopping and errands within the guidelines of the written plan of care.

11. Attends in-service programs according to agency guidelines, as directed.

12. Adheres to Private Duty and Home Health customer service expectations and reviews the care plan and client summary prior to visit.

13. Other duties as assigned

Position Qualifications

1. Educational: Must be 18 years of age.

2. Licensure: Valid driver's license and valid insurance.

3. Experience: At least one year of caregiving experience.

Takes oral and written instructions well; can read and write. Works in a team responsibly and independently without direct supervision. Records observations and activities. Good communication skills.

4. Knowledge and Abilities:

a. Demonstrates knowledge and skills necessary to provide care to and

communicate with individuals over the life span.

b. Shows an interest in and an aptitude for working with the sick, disabled or

elderly in their homes.

DEGREE OF TRAVEL

Home visits daily. Periodic office meetings. Must have reliable transportation and agency required liability insurance.

DEGREE OF DISRUPTION TO ROUTINE, OVERTIME

Must be able to adapt to frequent schedule changes, based on patient needs, staffing, weather and environmental conditions and new patients.
