We are seeking a dedicated and energetic Home Health Aide Team Lead to join our compassionate care team. Duties

Sta? Support & Coverage. Provide in-home care services during caregiver call-outs or sta?ng shortages. Introduce new home health aides to clients during their ?rst shift to ensure a smooth transition and build trust. Travel & Flexibility: Travel to client homes or o?ce locations as needed across service areas. Technology & Training: Train new and existing sta? on how to use Auxocare (or other scheduling/documentation systems). Troubleshoot technical issues related to care documentation or communication apps. Administrative Support: Assist the administrative team with ?ling and organizing employee documentation. Supervise and mentor a team of home health aides, providing guidance on patient care procedures and best practices Coordinate daily schedules to ensure timely and efficient delivery of services to clients. Assist with activities of daily living (ADLs) such as bathing, dressing, grooming, meal preparation, and mobility support using equipment like Hoyer lifts or wheelchairs Ensure compliance with healthcare regulations including HIPAA privacy standards and documentation protocols in electronic health record (EHR) or electronic medical record (EMR) systems. Maintain accurate medical records, medication logs, and documentation of care provided in accordance with agency policies. Promote a safe environment by adhering to infection control procedures and maintaining cleanliness during home visits

Skills

Strong knowledge of medical terminology, dementia care, Alzheimer's care, hospice care, and memory care techniques Experience working with people with developmental disabilities or disabilities in general within home or community settings Proficiency in EMR/EHR systems for documentation and reporting purposes Excellent patient observation skills with the ability to identify changes in condition promptly Ability to assist with complex tasks such as, catheter care, Hoyer lift transfers, and vital signs measurement Compassionate caregiver with experience in home health care, senior care, long-term care environments, assisted living facilities or nursing homes Knowledge of HIPAA regulations to ensure patient confidentiality at all times Effective communication skills for working with patients, families, healthcare teams and team members Ability to handle multiple priorities efficiently while maintaining attention to detail in documentation and patient safety

Pay: From $37,440.00 per year
