Overview

We are seeking compassionate and dedicated Home Health Aide to provide essential support to individuals in their homes. As a Home Health Aide, you will play a vital role in enhancing the quality of life for patients by assisting with daily living activities and ensuring their comfort and safety. This position requires a strong commitment to patient care, attention to detail, and the ability to work independently while following care plans.

Duties

Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and meal preparation Monitor and document vital signs and changes in patient condition Provide companionship and emotional support to patients Administer medications as prescribed and ensure proper medication management Assist with mobility using equipment such as Hoyer lifts when necessary Maintain cleanliness of the patient's living environment, including laundry and light housekeeping tasks Implement care plans developed by healthcare professionals, ensuring adherence to HIPAA regulations Provide specialized care for patients with conditions such as Alzheimer's, dementia, or developmental disabilities Document patient care activities accurately in EMR or EHR systems

Requirements

Previous experience in home health, assisted living, or long-term care settings is preferred Knowledge of medical terminology and ability to perform basic medical documentation Certification in CPR and first aid is highly desirable Familiarity with catheter care and hospice care practices is a plus Strong observational skills for monitoring patient conditions and reporting changes promptly Ability to work effectively with individuals with disabilities or special needs Excellent communication skills and a compassionate approach to caregiving Must be able to follow instructions accurately while working independently
