We are a dynamic home health agency committed to providing compassionate, high-quality nursing care in patients' homes. We serve a diverse population of clients with chronic and acute health needs, and you will be an integral part of our weekend care team.

Responsibilities

Provide home health nursing assessments, care planning, and implementation for patients in their homes. Administer medications, coordinate with physicians, monitor vital signs, manage wounds, and educate patients and families Document all nursing care according to agency policy and regulatory requirements. Communicate effectively with the weekday clinical team to ensure continuity of care. Provide patient education; make appropriate referrals based on identified patient needs that will maximize independence and patient/family participation Collaborates with the physician and other members of the health care team to ensure care is safe, effective and coordinated. Complete all OASIS time point assessments timely as per agency parameters. Adhere to professionalism safety protocols, infection control, and patient confidentiality.
RN Requirements/Qualifications:

Bachelor's Degree in Nursing preferred

Minimum of one year of home care experience required.

Current license from Board of Nursing in Massachusetts.

Valid driver's license as well as reliable transportation.

Skills/Competencies:

Ability to work independently in the home care setting Demonstrates responsibility for self-direction Demonstrates ability to learn and utilize computer technology Demonstrates effective communication skills

Pay: From $45.00 per hour
