Job Summary


We are seeking a compassionate and dedicated Home Care Aide to provide exceptional in-home support to individuals requiring assistance with daily activities. The ideal candidate will possess a strong commitment to patient care, excellent observational skills, and experience working with diverse populations, including seniors, individuals with disabilities, or those receiving hospice or memory care. As a vital member of our caregiving team, you will deliver personalized care that promotes independence, dignity, and comfort in the home environment. Responsibilities


Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support. Provide companionship and emotional support to foster a positive and engaging environment. Monitor and observe patients' conditions, noting any changes or concerns related to health or behavior. Administer medications accurately following prescribed instructions and document medication administration in medical records or electronic health record (EHR) systems. Support clients with meal preparation and nutritional needs while adhering to dietary restrictions and medical guidelines. Perform light housekeeping tasks such as cleaning, laundry, and maintaining a safe living space to promote hygiene and comfort. Utilize medical terminology appropriately when documenting patient information or communicating with healthcare professionals. Operate medical equipment such as Hoyer lifts for safe patient transfers and assist with catheter care or other specialized procedures as trained. Record vital signs regularly--such as blood pressure, pulse, temperature--and report any abnormalities promptly. Maintain compliance with HIPAA regulations to ensure confidentiality of patient information at all times. Assist with hospice or dementia care plans by providing specialized support tailored to each individual's needs. Work effectively within home & community care settings, demonstrating flexibility in live-in employment arrangements when required. Collaborate with healthcare teams to ensure continuity of care and accurate medical documentation.

Experience


Previous experience in home health care or assisted living environments is highly desirable. Demonstrated ability to work with people with developmental disabilities, Alzheimer's disease, dementia, or memory care needs. Experience working in nursing homes or long-term care facilities provides valuable insight into complex patient needs. Familiarity with EMR (Electronic Medical Records) systems and EHR systems for accurate documentation is preferred. Knowledge of medical terminology, medication administration protocols, CPR certification, and hospice care practices enhances your capability to deliver quality care. Hands-on experience operating medical equipment such as Hoyer lifts or performing catheter care is advantageous. A background in caring for seniors or individuals with disabilities--home & community care experience--strengthens your suitability for this role. Strong caregiving skills combined with excellent communication abilities ensure effective interaction with patients and team members alike. Pay: $16.00 - $20.00 per hour

Expected hours: 24.0 - 50.0 per week
