As a contracted provider for the Indiana Department of Child Services (DCS), NGBP upholds the highest standards of care by delivering evidence-based, family-driven services that are both equitable and outcomes-oriented. Our interdisciplinary team--comprised of licensed clinicians, certified case managers, peer mentors, and youth facilitators--collaborates to meet DCS performance expectations while remaining rooted in community voice, professionalism, and cultural humility.

Position Summary



The Home-Based Family Case Manager is a vital frontline professional tasked with delivering trauma-informed, family-centered case management services within the guidelines and service standards of the Indiana DCS. This position supports the preservation, reunification, and stabilization of families through in-home interventions, goal setting, and consistent client engagement.

This role requires excellent documentation, time management, and the ability to adapt services to complex family dynamics. Candidates should be familiar with child welfare practices and possess a commitment to ethical, culturally appropriate service delivery.

Key Responsibilities



Conduct comprehensive intake assessments using DCS tools (e.g., CANS, ANSA, AAPI-2, NSCS) Co-develop individualized service and safety plans with families and referring FCMs Provide weekly in-home visits focused on parenting, conflict resolution, trauma recovery, and life skills Facilitate referrals to community-based resources and track client engagement Attend Child and Family Team Meetings (CFTMs), court hearings, and team supervision sessions Submit monthly documentation by the 10th using DCS-approved formats Maintain Medicaid-compliant billing under appropriate MRO codes Begin services within 48 hours of referral and adhere to all DCS timelines Participate in biweekly individual supervision and peer shadowing for professional growth

Required Qualifications



Candidates must meet

one

of the following educational and experiential pathways:

Option 1:

Bachelor's degree in Social Work, Psychology, Human Services, or related field

Option 2:

High school diploma or GED

plus 4 years

of relevant direct service experience with families or children

Additional Requirements:



Experience with Indiana Practice Model and familiarity with KidTraks preferred Valid Indiana driver's license, reliable vehicle, and proof of insurance Ability to complete DCS onboarding and MRO training within 60 days of contracting
