Job Description

Overview

The Master Lease Case Manager is responsible for overseeing the supportive services and case management component of a master lease housing program. This role ensures that tenants under the master lease are stabilized, supported, and able to maintain housing through direct case management, coordinating with property management, and facilitating access to community resources.

Key Responsibilities

Manage a caseload of program participants (households or individuals) leased under the master lease agreement. Conduct intake assessments, including identifying barriers to housing stability (financial, health, legal, behavioral, etc.). Develop individualized support plans with participants, set measurable goals, monitor progress, and adjust plans as needed. Connect tenants to necessary supportive services (mental health, substance abuse, employment, financial literacy, etc.). Coordinate with property management/landlords to ensure lease compliance, timely rent payments, maintenance requests, and help resolve tenant/property issues. Monitor and assist with participants' benefit enrollment (e.g., public assistance, healthcare, food programs). Advocate for clients in interactions with service providers, landlords, and community agencies. Maintain up?to?date case documentation, including service plans, case notes, outcomes, and required reporting. Ensure compliance with program guidelines, funding requirements, and relevant laws/regulations. Participate in regular case conferences, team meetings, and trainings. Perform outreach to engage prospective participants, coordinate move?ins, and provide follow?up. Help with crisis intervention, as needed.
Qualifications

Required:



Bachelor's degree in Social Work, Human Services, Psychology, or a related field; or equivalent experience. Experience in case management, especially in housing or homelessness services. Knowledge of community resources, social service systems, benefits programs. Strong interpersonal and communication skills. Ability to work with diverse populations, including individuals/families with complex needs. Good organizational skills; ability to manage multiple cases, deadlines. Valid driver's license, reliable transportation.

Preferred:



Experience with master leasing programs or similar housing subsidy models. Experience in Michigan or familiarity with local Holland area resources. Familiarity with trauma?informed care or housing first approaches. Bilingual (English / Spanish or other) skills.
Working Conditions

Full?time position; may require occasional evenings or weekend work depending on client needs. Some travel locally (within Holland and surrounding areas) for home visits, meetings. Office?based work combined with field work.
Performance Measures / Success Indicators

Housing stability: e.g. percentage of tenants who successfully maintain tenancy over 12 months. Reduction in evictions or lease violations among participants. Client satisfaction and participant progress toward goals. Timeliness/completeness of case documentation and reporting. Responsiveness to property management concerns.

Salary range:

$35,000-$40,000/year
