Job Summary

The High-Risk Youth Coordinator conducts service navigation for at-risk youth and their families. Using restorative and trauma-informed practices, the Coordinator will support the Youth Outreach Team's ability to establish and maintain strong relationships with young people exhibiting runaways and other high-risk behavior. Responsibilities focus on referral services for at-risk youth and include the support and encouragement necessary to help youth use those referrals. The Coordinator facilitates youth and family connection to partner agencies within the various service systems as needed. Youth and their families are supported to find opportunities, solutions, and resources to address their social, behavioral, emotional, educational, and employment needs within the community.




Youth outreach activities could occur off-site in high-priority communities and/or neighborhood locations or settings. Tasks will include the following but are not limited to attending multi-disciplinary meetings and engaging in case conferencing, following up on calls for service involving youth, working with the Juvenile/Adult Justice partners to identify and deliver services to high-risk youth and their families, communicating with multiple stakeholders to identify long-term solutions for youth and families, coordinating with partner agencies to support mentorship needs, and helping to identify gaps in service to youth and their family members. This role will frequently interface with various contacts, including members of law enforcement, juvenile diversion, the school district, probation, public officials, community partners, etc... The Coordinator has a variety of responsibilities and requires the ability to maintain a high degree of sensitivity to confidential matters and be flexible with changing priorities and assignments.




Essential Duties and Responsibilities:



Work with community partners to support response to missing, run away, and truant youth, including connecting parents/guardians to services that can help address the underlined reasons why youth are running away.


Engaging with aspects of the juvenile justice system to understand workflow, barriers, strengths, and referral opportunities; connecting juvenile services with community-based services to better support youth and their families. Perform community engagement and outreach into neighborhoods affected by violence in partnership with community partners. Engage in a system of collaborative communication to ensure follow-up on youth identified as being high-risk. Manage the Handle With Care (HWC) Program to include training all law enforcement agents, maintaining and updating training materials, following up with School Resource officers, tracking HWC notifications, and connecting youth identified as high-risk to supportive services. Maintain tracking of Youth Outreach Team referrals, follow up activity with youth/families, and reports to patrol supervisors providing updates on referred youth. Facilitate, track, and report on Youth Outreach Team activity to supervisors, department leaders, community partners, and stakeholders. Attend meetings, case conferences, coordinated access meetings, training workshops, and community meetings as assigned. Assists in administrative duties, such as completing forms, gathering and transporting materials, data entry, or other similar support activities. Participates in professional training activities relevant to job-related duties Any other duties as assigned.


Qualifications:



Bachelor's Degree from an accredited university or college preferred.


Minimum of two years of community outreach/relations/youth mentorship experience in a professional capacity or a combination of education and experience deemed acceptable to the hiring authority. Ability to communicate effectively, orally and in writing. Ability to respond to work and rapidly evolving situations in the field. Ability to understand statutes, municipal codes, and legal issues relative to work supporting community initiatives. Ability to respond to problems and complaints creatively. Ability to diffuse and resolve conflicts with emotionally-charged individuals in stressful situations. Ability to provide excellent customer service. Ability to prioritize multiple duties and to work with interruptions. Ability to use a computer and related software to perform the position's essential functions. Ability to maintain confidentiality and exercise discretion and judgment in dealing with sensitive or confidential information.


Work Environment:



While performing the duties of this position, the incumbent is regularly required to sit, stand, reach with hands and arms, talk, and hear. Most of the work is completed at an assigned desk station in front of a computer. The incumbent is occasionally required to walk; balance; stoop, kneel, crouch, or crawl. The incumbent must regularly lift to 10 pounds, frequently lift and move up to 25 pounds, and rarely lift and move up to 50 pounds. Specific vision abilities required by this position include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.
