Travel within assigned and neighboring counties requiredWe are looking for a dedicated
Enrollment Case Manager (ECM)
to assist individuals applying for community-based services. This field-based role requires conducting assessments and supporting applicants through the enrollment process.Key Responsibilities:
Conduct in-home intake visits and facilitate eligibility determinations for community-based services Assist applicants with completing and submitting applications, selecting service providers, and ensuring a smooth enrollment process Build and maintain relationships with healthcare providers, community organizations, and stakeholders Provide support to applicants facing delays or obstacles during the enrollment process Attend regular team meetings and report availability Occasionally lift and carry items up to 25 pounds
Requirements:
Minimum of one year of experience in public or private social work At least two years of casework experience, including one year of conducting functional assessments Bachelor's degree with at least 12 credits in social sciences (e.g., sociology, psychology, gerontology) Must have a valid driver's license and access to reliable personal transportation (public transportation is not permitted)
Job Type: Contract
Pay: $20.50 per hour
Work Location: In person
