Camellia Place Assisted Living and Memory Care is looking for Georgia Licensed Registered Nurse (RN) with experience working with older adults in a seniors housing setting. DON\'T be the only nurse in the building! At Camellia Place, the Health Services Director manages a team including a Health Services Manager (LPN) and a Health Services Coordinator (CMA) who work in collaboration managing team members and coordinating resident care needs.

Job Overview:

The Health Services Director (HSD) a licensed nurse, directly supervises, manages and coordinates the Health Services department, team members, and the care and services provided for residents in the community.

Job Qualifications:

? Education:

? A Registered Nurse preferred with at least one year of full-time experience as a successful supervisor or manager in an acute care, long term care (LTC), personal care home (PCH), or assisted living community (ALC). OR

? A Licensed Practical Nurse with at least five years of experience in an acute care, LTC, PCH, or ALC. At least one year of which has been in a successful, full-time role as a supervisor.

? Experience:

? Proven clinical expertise and nursing judgement in geriatric populations.

? Proven experience greater than 1 year as Director of Nursing or similar role in other long-term care relative managerial position.

? One year + experience supervising others to include but not limited to: managing/ coordination of resident care and service needs, education, and training, setting performance expectations, setting and managing schedules, coaching and mentoring team members, promoting teamwork, managing change, providing performance feedback, delegating, resolving conflicts, managing expense and budgets, etc.

? Other:

? Experience in developing strategies and plans and execution with outcomes.

? Strong understanding of budget, expenses and labor management and measures of performance

? An analytical mind capable for out-of-the-box thinking to solve problems.

? Outstanding organization and leadership abilities

? Excellent communication (oral and written) and public speaking skills

? Understanding of balance with healthcare and hospitality model for senior living

? Must be physically and mentally able to always perform the duties of the position.

? Background checks, drug screens, and driving records:

? Must have active nursing license in good standing cleared to practice in Georgia.

? Must be cleared upon hire and ongoing from all background check requirements to work in an assisted living community in compliance with all state and local rules and regulations and within company requirements.

? Must be clear from drug or alcohol use upon hire and/or upon random or scheduled request by the company.

? Must maintain a clean driving record to operate company vehicle(s).

? Must be cleared by physical examination (GA)

Requirements: Essential Responsibilities

Resident Care Services

The HSD is responsible for the overall coordination and delivery of quality resident care and services in compliance with State of Georgia assisted living community (ALC) rules issued by the Department of Community Health (DCH), as well as other rules and laws applicable to an ALC in Georgia. Examples of additional applicable rules and laws include but are not limited to those from the Department of Public Health (DPH), Georgia Food Code, State Fire Marshal rules for ALCs, applicable county and local laws and rules, and rules determining Emergency Operations Preparedness (EOP).

Ensures resident evaluations and service plans are completed and kept current in accordance with regulatory requirements and community policies and procedures.

Ensures resident clinical changes in condition are properly documented and coordination of care is maintained at all times.

Perform nursing assessments and/or treatments as needed and within scope of nursing license

Communicates with residents and/or responsible party regarding changes to resident care and/or condition.

Maintains professional relationships with flow of communication with vendors for proper coordination of care such as pharmacy, physicians, Home Health and other ambulatory service providers.

In cooperation with the Executive Director and Community Liaison provides an evaluation of each resident applicant\'s care and services needs to determine if that applicant meets DCH standards for admission to an ALC, and if the applicant\'s needs can be met by the community\'s existing staff.

Determines the Level of Care (LOC) needed by each resident for the purpose of calculating the LOC services fee based upon the Resident Evaluation and Service Plan developed.

Ensures all resident records are established prior to resident move-in, are continually maintained throughout the resident\'s time living at the community and are retained in the records archive for at least 3 years after the resident leaves our community.

Provides education and guidance regarding disease process for residents and families within scope of nursing licensure.

Ensures health services team members are adequately informed of new and changed resident service plans.

Investigates with conclusions for final outcomes related to accident, injury, incident reports.

Ensures community Infection Control protocols are upheld and trained for all team members.

Maintains awareness of each resident\'s usual appearance and behavior patterns, level of functioning, and general condition so changes in a resident\'s condition can be identified and appropriately addressed.

Department

Demonstrates knowledge of, and compliance with state rules for ALCs and Memory Care Centers as well as Community Policies and Procedures

Ensures that a departmental staffing schedule is maintained to meet Community needs and state mandates and is posted timely. Ensures there are effective mechanisms in place to secure replacements for unplanned team member absences or other types of teammate shortages. The schedule is planned out at least 30 days in advance.

Directs the Health Services Team in compliance with the established goals and priorities and provides feedback to the Executive Director as to progress, and whether adjustments to the plan may be needed.

The HSD works in cooperation with other departments to ensure the health, the safety, and the satisfaction of our residents and their families. Effectively uses resident, family, and teammate feedback to measure success of goal achievement.

Coordinates, prepares, and participates in a monthly Quality Assurance review with the Executive Director and designees.

Facilitates appropriate or necessary communications and problem solving of concerns and grievances expressed by residents, their responsible parties and families, their healthcare providers, home health agencies and hospice agencies that have been contracted by the residents, and interdepartmental relationships and functions of the community that affect the Health Services Department. The HSD facilitates and ensures the satisfactory resolution of expressed or observed concerns, grievances, or Complaints, and documents the course of events and resolution utilizing the Concerns/Grievance Form with the Executive Director

Oversees the safe storage and administration of resident medications as well as all other aspects of the medication administration program in accordance with policies and procedures approved by the Governing Body.

Responsible for monitoring Controlled Drug count accuracy and investigating any discrepancy. Discrepancies are documented on a Medication Exception Report form and are immediately reported to the Executive Director.

Performs the routine MAR audits in collaboration with Pharmacy consultation.

Personnel

Participates in recruiting and retention efforts for the department including but not limited to screening applicants, interviews, hiring, coaching, performance evaluations, disciplinary actions, team recognition, individual evaluations, etc.

Creates and manages department schedules posted in advance. At times, if unable to find adequate coverage, provides coverage of shift needs and of the residents care which may include managing medications, providing assistance with ADL\'s and other role requirements.
