Job Description

Summary



To abstract patient charts with accuracy and in a timely manner. As well as accurately and timely perform other functions within the Medical Record Department, as assigned by the Director.

The Records Clerk is expected to perform the following essential functions:



Review discharge charts for completeness of documentation, complete signatures, ancillary reports, etc. with forwarding of incomplete charts to various parties in a timely manner. Review and permanently archive patient census. Forward discharge chart data to attending physician in timely manner. Chart all reports and documentation to discharge charts on a daily basis. Compile monthly State Vital Statistics Death report in designated time frame. Retrieve previous chart information and place on new admissions daily. Maintain accurate record of charts released from the medical record department and assures their prompt return. Assures admission and discharge time accuracy in the computer system for floor patients. Possess knowledge of the software and hardware of the optical scanning system and facility wide information management system and utilize it appropriately. Possess knowledge of other duties within the department and assist other employees when needed. Demonstrate competed general office procedures (i.e., filing, copier, fax, phone, etc.). Possess thorough knowledge of and follow confidentiality and HIPPA guidelines. Demonstrate desire for self-improvement by attending in-service education and meetings. Comply with hospital dress code and other policies as listed in the BMH employee handbook.



Required Skills, Qualifications, and Competencies



Must possess a high school diploma or equivalent. Must have understanding of medical terminology. Must possess understanding of word processing. Must possess ability to complete assigned clerical tasks in accurate manner with reasonable speed. Must possess ability to work well with public, co-workers and medical staff in a competent and courteous manner.

Physical demands



Ability to perform various motor skills such as; standing or sitting for long periods of time, walking, lifting (up to 10 pounds) using proper lifting techniques, stair climbing, reaching, bending, twisting, pushing, pulling, squatting, grasping and arm-hand coordination on a continuous basis.
